Permission Form & Liability Waiver

This form needs to be completed by any player under 18 or vulnerable adult wishing to participate in play with a senior team.

Regardless of a young person’s size or sports ability, their status and potential vulnerability as a young person should not be ignored. As well as more obvious factors such as the physical implications of mixed age participation, there are several other specific issues that need to be considered when mixed aged activities involve children, young people or adults. These should be discussed and agreed with the young person, their parents/guardian and club staff beforehand. In addition to the completion of the waiver form clubs may wish to have other written agreements covering all arrangements. The YBL

It is the responsibility of the club to ensure all appropriate considerations have taken place before making the decision to permit U18s to play in a senior team. This should include considerations regarding physical safety, supervision, changing/showering arrangements, codes of conduct, and clarity on how concerns are raised.
